Unstageable Pressure Injury
This open wound presents with full-thickness skin loss as well as tissue loss. However, the true extent of damage cannot be confirmed because the wound bed is obscured by necrotic tissue (slough is typically yellow, while eschar is black). Wound can also present as a blood-filled blister.
