Unstageable ​Pressure Injury

This open wound presents with ​full-thickness skin loss as well as ​tissue loss. However, the true ​extent of damage cannot be ​confirmed because the wound ​bed is obscured by necrotic ​tissue (slough is typically yellow, ​while eschar is black). Wound can ​also present as a blood-filled ​blister.
